This form is a 4-part carbonless repair order with consumer disclosure requirements for the state of California. This form has room for additional entries and the second part is clearly identified as a customer invoice. Use this form in states with stronger consumer disclosure guidelines. The form includes the "you may choose another facility" and "no implied warranty" disclaimers and boxes for the disposition of old parts. The 4th copy is a hard manila index stock.
