This is a two sided form, with the back designed to list all beverages sold and charges applied. This restaurant check also has a removable stub that is used as a receipt. These guest checks can be padded into 50 forms per pad for an additional charge of $3.00 per 1000 forms. It is available with numbering or no numbering. If requested numbered there is no choice of starting number.
