A standardized work order designed for the larger repair jobs. It contains more lines on which to describe the service to be completed, the accessories to be added and listing of parts required to complete the repair. The back of the form contains an additional parts used section plus a time card section for accurate on/off time of the job. A signature blank, authorizing the work to be completed is provided on the front of the form.
